Palliative Care
Regardless of the patient’s age or stage of illness, CoxHealth at Home’s palliative care physicians and nurses can help provide relief from symptoms, pain, and the stress patients and their families experience while living with serious illness. Working in concert with the patient’s team of physicians and nurses, our palliative care program is a consultation service that provides an extra layer of support the patient so often needs. Palliative care, which is covered under Medicare Part B, MO HealthNet, and some private insurance, is for those who:
- Have a life limiting illness of one to two years
- Might still require and want treatment toward a cure
- Desire an enhanced quality of life
- Would like emotional, physical, and spiritual support
- Need pain management and control of distressing symptoms
Our Palliative Care Team
Social Worker
- Completes patient admission
- Conducts weekly check-up call
- Provides counseling and emotional support as needed
- Provides support for any community resources needed
Nurse Practitioner
- Conducts initial nursing physical assessment
- Creates plan of care
- Conducts medication review
Case Manager
- Provides monthly visit (and as needed) to assess physical condition
- Teaches medications and disease process
- Advocates for health care needs
- Provides support for symptom management
- Communicates with medical providers
- Communicates with other CoxHealth at Home departments
- Conducts medication review
Chaplain
- Provides spiritual and emotional support
- Provides advanced care planning
- Provides family counseling
Music Therapist
- Helps with many illnesses using music
- Provides uplifting and peaceful music
- Allows for participation, if desired
Medical Director
- Conducts home visits as requested by the case manager
- Consults with medial providers
- Supports the team as a professional resource

We Honor Veterans
We are proud partners in the We Honor Veterans program, providing individualized, respectful end-of-life services for former military members. These include programs, resources and benefits information tailored specifically to veterans and their families. The program is a joint effort of the National Hospice and Palliative Care Association and the Department of Veterans Affairs.
Continuing Touch
Continuing Touch serves patients who have improved in the hospice or palliative care programs. Chaplains and social workers stay in touch with the patient and family, and they are available to assist when needed.
Bereavement Services
Bereavement Services are free to families served by Oxford as well as to the community. Seminars and individualized support groups are available to those who have suffered a loss, and professional staff maintain contact to help families work through the grief process.
