Private Home Care Services
At CoxHealth at Home, we help our clients age in place. With our private home care services, we work directly with our clients and their families to design individualized care plans that help improve quality of life, while providing safety and independence at home. Our customized plans focus on assistance with the things that have become more difficult for clients to do on their own. We continue to modify our private home care services as needs change over time. Some examples of private home care services include:
- Household duties
- Shopping and errands
- Personal care assistance
- Comprehensive medication management
- Transportation to church, doctor’s appointments and hair appointments
- Assistance with hobbies such as gardening, antiquing, dining out, reading, etc.
- Lifeline Personal Emergency Response System for peace of mind
Depending on your needs, the private home care services plan we design for you or a family member could include CoxHealth at Home’s Lifeline personal emergency response system, our Memory Care Program that provides expert care to clients suffering from Alzheimer’s and other dementias, and/or remote telemonitoring.
Lifeline
Trusted by more hospitals, doctors, professional caregivers, and families, CoxHealth at Home’s Lifeline is a perfect addition to your private home care services, as it provides security in case of a medical emergency, a fall, a house fire, or anytime you are in need of immediate assistance. But Lifeline is not just a personal response system for emergencies.
Subscribers wear a device that offers help at the push of a button, or, when equipped with AutoAlert, detects falls and calls for help automatically. Oxford Lifeline can travel with you anywhere in the United States, and it can even predict the risk of a fall, allowing concerns to be addressed before they become emergencies.
All CoxHealth at Home Lifeline services provide help 24 hours a day, 365 days a year, and we offer the only locally managed program in our area.
Memory Care
CoxHealth at Home’s Memory Care program is another powerful program available through our private home care services and uses an innovative and family-centered approach that teaches caregivers how to care for their loved ones at home.
Our specialized caregivers receive ongoing education in dementia care, which allows them to train family members and caregivers on how to help with basic daily activities. As a result, family members and caregivers are educated on how to handle challenging behaviors, learn how to manage medications, and more.
Remote Patient Monitoring
Reducing Trips to the Hospital
For clients with recent hospitalizations or who have experienced frequent emergency room visits for existing chronic conditions, Cox Heath at Home also provides remote patient monitoring. On average, this service has resulted in a 57% reduction in hospitalizations and emergency room visits, and a 65% reduction in days spent in the hospital.
If you or a loved one has a condition requiring frequent monitoring of vital signs, a chronic illness requiring self-monitoring, or a history of poor compliance with diet or medications, then remote patient monitoring might be beneficial private home care services component for maintaining optimal health.
Setting up our remote patient monitoring service is easy. One of our experienced staff members will come to your home to set up our system, show you how it works, and answer any questions you might have. With remote patient monitoring, you have peace of mind that a trained professional is monitoring your health daily and communicating vital information back to your physician, or to the physician of a loved one.
